This is why the Clamp setting was replaced with separate Direct and Indirect controls in newer versions of Blender: it seems to be the indirect bounces that are the main cause of fireflies, so this way you can clamp (so to speak) down on them without losing too much light from the scene.
But of course indirect bounces add much of the softness to shadows, so you don’t want to suppress them too much.
In my experience, they happen when you add a nonzero roughness setting to a glossy or refraction node (or glass, which is effectively a combination of both). They are the result of numerical instabilities in trying to calculate the rendering formulas.

best way to get rid of fireflies is using seeds, make a lot of images with different seeds, than use nodes to "blend" them in one. I think there is a tutorial that explains how to do that.
I found that for the dark scene I am doing having o.46 got me no fireflies and still held that realism. This clamp is REALLY bloody odd setting – there is no predicting on how it behaves on renders. I changed on material behind the camera (reflector) and I had to change it from o.46 to 10.5… Yet the overall picture was the same.

Can also render same image twice with different seed settings under the sample menu. Then combine those images using a darken layer mixer in post. Or a despeckle or dust & scratches noise filter in post also works fairly well.
As for what I find to cause the most fireflies? Velvet shader under glass with strong lighting hitting it. (YMMV)

I like tutorial will you please make a tutorial using Cycle on how to keep the fluid inside a cup, I have tried to follow other tutorial about fluid and all I get is the domain fill of water and the cup is empty.

that material on the cgcookie ship. cmon lol. you could have gave that a better mat. and that simple scene for reflections just blendswap a scene. presentation is everything. but im just nitpicky lol. cool video, keep at it bra.
